Athlete Questionnaire
Welcome to the Onondaga Community College athletic team. At Onondaga, we hope you have a safe and injury free experience while participating in athletics. Unfortunately, accidents and injuries do occur in athletics. We at Onondaga want to ensure that you will receive the best possible care and in a timely fashion if an injury shall occur while participating in athletics. In order to make sure you receive the best possible care we need some medical information from you prior to your participation.
Below you will find links to forms that you must print and fill out entirely and to the best of your knowledge. This information is required by both the NJCAA and Onondaga prior to your participation in athletics. If these forms are not filled out completely you will not be allowed to try out or participate in Onondaga athletics.
This first link listed below is for First Year Student Athletes/Transfer Student Athletes. This link will bring you to the first year student athletes/transfer student athlete medical history form. This medical history form should be completed and signed by the student athlete. Circle “yes” or “no” for each question and please provide dates and details when specified. The last page of the medical history form is for your Sport Physical, which must be completed by a qualified health care professional licensed to administer physical examinations. The exam should be dated after May15th, 2012 and must clearly state that you may participate in intercollegiate athletics with no limitations. If you are currently under the care of a physician, physical therapist, etc. for an injury or illness, please have that health care provider fill out a referral form. A written statement by them is required prior to your participation.
The second link listed is for Returning Student Athletes. This link will bring you to the returning student athlete medical history form. This medical history form should be completed and signed by the student athlete. Circle “yes” or “no” for each question and please provide dates and details when specified. The third link listed below is the Assumption of Risk/Information Release Authorization Form. This link will bring you to the assumption of risk/information release authorization form. This form should be read and signed by the student athlete. If you are under eighteen, a parent or guardian should also sign this form.
It is mandatory at Onondaga you have personal insurance in order to participate in athletics. A photocopy (front and back) of your current medical insurance card must be on file prior to your participation.
All medical information is strictly confidential and will be used as an aid in providing you the best health care possible while you are a student athlete at Onondaga.

If you need any medical information sent to another school or doctor’s office you must print out the Medical Release Form in order to acknowledge the release of your personal medical information. The medical release form must be completed and signed by yourself and a witness. The form must be returned to me before any information can be sent out.
